Study Abroad through University Agreement Procedure

The types of documents to be submitted and the procedure for submitting them may change depending on the academic year. For provisional applications, final applications, and pre-departure documents, the forms for previous years are posted for reference (only available to current students Musashi University). The latest information and application forms for the current academic year are available on the 3rd.SPlease check.

Provisional application documents

■ Brief Self-Introduction (1 A4 self-introduction in the language of the host country and 1 A4 in Japanese)

■ Doctor's certificate *Must meet the items specified in "Study Abroad through University Agreement Medical Examination for Application"
■ Study Abroad through University Agreement Request for Letter of Recommendation for Applicant Students
■ Copy of proof of language proficiency (Examinee Copy of official language proficiency certificate with name)
■ List of courses registered for this year (spring and fall semesters) (print out 3S "Student Timetable")
■ Printout of the page with the "Graduation Requirements" in the course requirements
■ Copy of passport (page with name, date of birth, expiration date, etc.)
■ Ranking table of desired universities to study abroad

II. Documents to be submitted before departure

■ K-1 Musashi University Study Abroad Scholarship Application Form (for undergraduate students)
■ K-2 Flight Schedule Report and Visa Copy
■ K-3 Overseas travel insurance statement
■ K-4 Report of overseas residence and mobile phone number
■ K-5 consent form
■ K-6 Guarantor Passport Number Report
■ Goal sheet/Summary sheet

III. Items to be submitted after arriving at the destination

■ Submitting a residence notification to a Japanese diplomatic mission (using the Ministry of Foreign Affairs' electronic system)
■ Monthly report Email to the Global Education Center and your academic supervisor
■ Interim report Email to the Global Education Center

Ⅳ Items to be submitted after returning to Japan

■ Submit a return notification to a Japanese diplomatic mission (via the Ministry of Foreign Affairs' electronic system)
■ Notification of completion of return to Japan
■ Image data of airline ticket stubs
■ Study Abroad Experience
■ 2 photos showing what studying abroad is like
■ Final Report
■ Goal sheet/Summary sheet
■ Credit Approval Application (Submit to Academic Affairs Division)
